This resource was developed from the results of a National Delphi Study that engaged the expertise of youth, family members/caregivers, clinicians and administrators to identify best practices, or core components, in mental health care transitions. In total, 26 core components of successful mental health transitions were refined and selected. The final list of components prioritizes patient-led care and collaborative decision-making at all stages leading up to and following the care transition. This resource can be used to develop transition protocols that best suit your organization’s context, resources, and stakeholder needs.
